None for a home education program itself. A declaration goes to the local public school only in the two circumstances RSA 193-A:3 names: to obtain access to public school programs under RSA 193:1-c, or on withdrawal from a public school.
Who actually reviews your child
No one. RSA 193-A:4 II: no superintendent, school board, principal, other district official, local official or state official 'shall propose, adopt, or enforce any policy or procedure governing home educated students' except policies for the RSA 193:1-c courses and programs.
What you must file
Nothing. There is no registration, no notice and no annual return to submit in New Hampshire. That freedom is real, and it carries a quiet catch: because nobody ever required a record, there is no file to reach for the day someone finally asks what your child has learned.

The part the paperwork does not cover
Filing on time is the easy half. The harder half is the year in between: showing what your child actually learned, in words the reviewer recognises, without turning your home into a school.
